504 Plan

This lesson will walk through how to complete a 504 Plan.

NOTE: You must have the permission for 504 Plans ( 504 Plan - Access) within Infinity in order to see and navigate to it. Please contact your System Administrator if you do not see the option and feel that you should have it.

NOTE: You must also have the permissions within the 504 Platform to View and Edit 504 Plans.

1. Select Special Services.

2. Select 504 Plan.

If you do not already have a student within your Student Ribbon in Infinity, it will take you to Search to locate the student you wish to work/view a 504 Plan for.

1. Any 504 Plans that have been started/published will display here.

2. To begin a new 504 Plan, select Create.

FYI: To navigate to a new Student, Caseload or the Activity Log, select the Hamburger Icon (Three horizontal lines).

1. Select the District the student is enrolled using the dropdown.

2. Select/type the Date of the 504 Plan Meeting.

3. Select the Time of the 504 Plan meeting.

4. Select the Purpose of the 504 Plan meeting.

5. Select Continue to Participants to continue through the 504 Plan process.

1. Select the Parent(s)/Guardian(s). NOTE: These come from Infinity so if there is nothing showing this means there are no contacts for this student within Infinity.

2. Select the District Representative.

3. Select the General Education Teacher.

4. Select Continue onto Invitation Letter.

NOTE: You may also add additional participants by selecting Add Participant.

1. Select Yes or No in regards to if you would like to document contact attempts for the 504 Plan meeting. NOTE: If you select Yes, then at least one contact attempt will be required to move to the next section of the 504 Plan.

2. Enter in the Parent/Guardian information shared in regards to their child's impairment and they feel this might impact major life activities. NOTE: To learn more about what is a Major Life Activity, select the ? Icon.

3. Select Continue to Student Needs to move onto the next section of the 504 Plan.

This portion of the Student's Need Page is to review the student's current plans, evaluations, etc. and determine if the student has an impairment and then if the impairment impacts one of the Major Life Activities.

1. Select either Yes or No in regards to if the student has a health plan. The purpose of this question is to let others know that there is a Health Plan in place that may need to be reviewed/considered.

2. Select either yes or No in response to, Based ont he evaluation information reviewed, does the student have a physical or mental impairment?.

*If No is selected, upon selecting Continue, the 504 Plan will move forward to the Notice/Consent portion of the 504 Plan process.

* If yes is selected, the system will move forward with determining if the student shows a need for a 504 Plan.

NOTES:

1. If the student does not have an Impairment , upon selecting Continue, the 504 Plan will move forward to the Notice/Consent portion of the 504 Plan process.

1. Select the student's Impairment(s) from the dropdown.

2. Provide a description of the student's impairment. Be sure to describe the condition/need that is presenting itself for the student.

3. Select either Yes or No in response to, Does the student's impairment substantially limit one or more major life activities? NOTE: To learn more about what is a Major Life Activity, select the ? Icon.

*If No is selected, upon selecting Continue, the 504 Plan will move forward to the Notice/Consent portion of the 504 Plan process as the student has been determined to be ineligible.

*If Yes is selected, the 504 Plan will continue to determine what MLAs are substantially impacted by the student's impairment(s) and where will these MLAs (Major Life Activities) need to be addressed for the student.

1. Select the MLA (Major Life Activity) from the dropdown.

2. Enter in the reason why this is a MLA (Major Life Activity)for the student and how it impacts them.

3. Enter in any current or previous data in regards to the student. This can include previous accommodations, modifications and services used/provided.

4. Select the area(s) where the MLA needs to be addressed in for the student.

5. To add additional another MLA (Major Life Activity), select +Add Major Life Activity.

6. To continue to Accommodations/Modifications/Services, select Continue.

1. Select +Add Accommodation.

1. OPTIONAL: To limit the results for the Accommodation dropdown, select the Category(ies) you would like to search within.

2. Select the Accommodation(s) the student requires. NOTE: This is a mult-select field BUT if the condition, location and Role(s) responsible are not the same, then select one Accommodation ONLY and then add additional Accommodations separately.

3. Enter in when the Accommodation should be used. Be sure to DESCRIBE the when/how/etc. as appropriate. The more visible you are with the condition, the easier it will be to implement the accommodation.

4. Select where the accommodation be provided. NOTE: This is a multi-select field.

5. Select the Role(s) responsible for providing the accommodation.

6. Select if the accommodation will being upon the implementation by selecting Yes or No. NOTE: If no is selected, a date selector will appear to input the Start Date.

7. Select if the accommodation will continue through the life of the 504 Plan by selecting Yes or No. NOTE: If no is selected, a date selector will appear to input the End Date.

8. OPTIONAL: To add another accommodation, select +Add Accommodation.

NOTE: At least one accommodation will be required IF Accommodation was selected as an area that would be addressing the students MLA within the Student Needs page.

1. Select +Add Modification.

1. OPTIONAL: To limit the results for the Modification dropdown, select the Category(ies) you would like to search within.

2. Select the Modifications) the student requires. NOTE: This is a mult-select field BUT if the condition, location and Role(s) responsible are not the same, then select one Modification ONLY and then add any additional Modifications separately.

3. Enter in when the  Modification should be used. Be sure to DESCRIBE the when/how/etc. as appropriate. The more visible you are with the condition, the easier it will be to implement the Modification.

4. Select where the Modification be provided. NOTE: This is a multi-select field.

5. Select the Role(s) responsible for providing the Modification.

6. Select if the Modification being upon the implementation by selecting Yes or No. NOTE: If no is selected, a date selector will appear to input the Start Date.

7. Select if the Modification will continue through the life of the 504 Plan by selecting Yes or No. NOTE: If no is selected, a date selector will appear to input the End Date.

8. OPTIONAL: To add another Modification, select +Add Modification.

NOTE: At least one Modification will be required IF Modification was selected as an area that would be addressing the students MLA within the Student Needs page.

1. Select +Add Service.

1. Select the Service the student requires.

2. Select the District the student is enrolled in.

3. Enter in when the Service should be used. Be sure to DESCRIBE the when/how/etc. as appropriate. The more visible you are with the condition, the easier it will be to implement the Service.

4. Select where the Service be provided. NOTE: This is a multi-select field.

5. Select the Role(s) and/or User(s) responsible for providing the Service. At least one role or User MUST BE SELECTED. NOTE: This is a multi-select field.

6. Select if the Service will being upon the implementation by selecting Yes or No. NOTE: If no is selected, a date selector will appear to input the Start Date.

7. Select if the Service continue through the life of the 504 Plan by selecting Yes or No. NOTE: If no is selected, a date selector will appear to input the End Date.

8. OPTIONAL: To add another Service, select +Add Service.

NOTE: At least one Service will be required IF Service was selected as an area that would be addressing the students MLA within the Student Needs page.

Expand or collapse content State and District Assessment

This page will allow you to add any Accommodations needed for State Assessments and/or for District Assessments.

If none are needed, select Continue to Comments.

Expand or collapse content Add State Assessment Accommodations

1. Use the dropdown to select the Subject Area(s) the Accommodation is needed for. NOTE: This is mult-select.

2. Select the Accommodation. NOTE: You may only select ONE Accommodation at a time. To add another one, select +Add Assessment.

3. Select if the Accommodation is Standard.

Add additional Accommodations by selecting, +Add Assessment.

1. Type the name of the District Assessment.

2. If the student shall not take the District Assessment, type the name of the Alternate Assessment the student will be taking in lieu of the District Assessment.

3. Select the Accommodation if appropriate/needed.

4. Select if the District Assessment is appropriate for the student.

5. Select if the Accommodations are needed for the assessment. If none are needed then nothing will be required in the Accommodation dropdown.

1. If the student will be graduating within the next two years, select Yes and enter in any key information that will be beneficial to know.

2. If any Major Life Activities had Other Considerations selected as an area where it will be addressed, these will populate here. If you would like them to populate within the box below, select Add to Comment.

3. Describe any other considerations for the student that have not previously been addressed.

4. Select Continue to Notice/Consent.

Expand or collapse content Preview/Publish the 504 Plan

1. Print the 504 Plan for review and to provide a copy to the parent(s)/guardian(s).

2. Preview the 504 Plan to ensure the information is correct.

3. To complete the 504 Plan, select Publish. NOTE: This will publish the 504 Plan and it will no longer be editable.
