504 MDR

This lesson will walk through how to complete a 504 MDR.

NOTE: You must have the permission for 504 MDR ( 504 MDR - Access) within Infinity in order to see and navigate to it. Please contact your System Administrator if you do not see the option and feel that you should have it.

NOTE: You must also have the permissions within the 504 Platform to View and Edit 504 MDRs.

1. Select Special Services.

2. Select 504 MDR.

If you do not already have a student within your Student Ribbon in Infinity, it will take you to Search to locate the student you wish to work/view a 504 MDR for.

1. Any 504 MDR that have been started/published will display here.

2. To begin a new 504 MDR, select Create.

FYI: To navigate to a new Student, Other Form (such as the 504 Plan), Caseload or the Activity Log, select the Hamburger Icon (Three horizontal lines).

Expand or collapse content Add Change of Placement Information

1.  Select the District the student is enrolled in from the dropdown.

2.  Select  either Yes,  No or There is not a change in placement at this time, in response to if the suspension was due to a serious and immediate threat... that led to the Change of Placement.

FYI:  If there is no change of placement but an MDR is still required, select, There is not a change in placement at this time.

Based on what you have selected in response to Change of Placement, different requirements will appear.

Was the Suspension due to a Serious and Immediate Threat to the Safety of the student or Others That Cannot be Mitigated by Other Means?

  1. Select the Parent(s)/Guardian(s) from the dropdown.  NOTE: These come from your Student Information System.
  2. Select the Date of the Offense.
  3. Select the Date the Notice to the Parent(s)/Guardian(s) for Change of Placement.
  4. Enter in the method for which the Notice will be sent.
  5. Enter in the number of days of the Suspension.
  6. Enter the Suspension Start Date.
  7. Enter the Suspension End Date.
  8. Provide the Code of Conduct(s) the student violated.
  9. Enter any comments.
  10. Describe the action that has/will occur as a result of the Code of Conduct Violation.
  11. Select Yes or No in response to if the MDR meeting has been scheduled yet?  NOTE: This will be reflected in the Notice of Change in Placement.

  1. Enter the MDR Meeting Date.
  2. Enter the MDR Meeting Time.
  3. Enter the MDR Meeting Location.
  4. Enter the name of the  District Contact.
  5. Enter the District Contact's Phone Number.
  6. Select the District Representative (this may be the same or different from the District Contact).
  7. Have the District Representative Sign the Notice of Change in Placement.  (It will add the date/time stamp once the signature is complete.)
  8. Print the Notice of Change in Placement.
  9. Select Continue to move onto the MDR.

  1. Enter the name of the  District Contact.
  2. Enter the District Contact's Phone Number.
  3. Select the District Representative (this may be the same or different from the District Contact).
  4. Have the District Representative Sign the Notice of Change in Placement.  (It will add the date/time stamp once the signature is complete.)
  5. Print the Notice of Change in Placement.
  6. Select Continue to move onto the MDR.

  9. Select Continue to move onto the MDR.

  1. Enter the name of the  District Contact.
  2. Enter the District Contact's Phone Number.
  3. Select the District Representative (this may be the same or different from the District Contact).
  4. Have the District Representative Sign the Notice of Change in Placement.  (It will add the date/time stamp once the signature is complete.)
  5. Print the Notice of Change in Placement.
  6. Select Continue to move onto the MDR.

1. Select Continue.

Expand or collapse content 504 MDR Meeting Information (NOTE:  This will only be required if the meeting has not been scheduled yet or you selected there was no Change in Placement)
  1. Enter the MDR Meeting Date.
  2. Enter the MDR Meeting Time.
  3. Enter the MDR Meeting Location.
  4. Select Continue to Participants.

1. Select the Parent(s)/Guardian(s). NOTE: These come from Infinity so if there is nothing showing this means there are no contacts for this student within Infinity.  You will want to speak to your key 504 contact to get them added to your student information system in order to be able to import them into Infinity/504 platform.

2. Select the District Representative.

3. Select the General Education Teacher.

4. OPTIONAL:  Add additional Participants.

5.  Verify/add/adjust the student's address.

6.  Select Continue to Invitation Letter.

1. Select Yes or No in regards to if you would like to document contact attempts for the 504 Plan meeting. NOTE: If you select Yes, then at least one contact attempt will be required to move to the next section of the 504 MDR.

2.  Select Yes or No in regards to if a Participant was in attendance or not.

3. Select Continue to Drug / Alcohol Use to move onto the next section of the 504 Plan.

This portion of the MDR is to review the requirements around Drug or Alcohol Use that are required as a part of the MDR process.

1. Select either Yes or No in regards to, Does the student currently engage in the illegal use of drugs or alcohol?

2. Select either Yes or No in response to, Is the student being discipline for the possession or use of illegal drugs or alcohol?

*If No is selected in response to BOTH questions, the MDR will continue to move forward to Considerations for Review.

* If Yes is selected in response to EITHER question, the process will move forward to Notice.

3.  Select Continue.

1.  Describe the behavior, in detail, that led to the misconduct.  Be sure to not use other student names, etc. within the description.

2.  Select/Enter the Date where All relevant information in the student's file was reviewed.

3.  Select the type(s) of information reviewed.

4.  Enter in the summary of the review.

5.  Select/Enter the Date where the student's 504 Plan was reviewed.

6.  Select the type of information reviewed.

7.  Enter the summary of the review of the 504 Plan.

8.  Select/Enter the Date of the Teacher observations of student.

9.  Select the type of information reviewed.

10.  Enter the summary of Teacher observations of student.

11.  Select/Enter the Date Parent(s)/Guardian(s) relevant information was obtained.

12.  Select the type of information reviewed.

13.  Enter the summary of relevant information provided by the parent(s)/guardian(s).

14.  Select Continue to Manifestation Determination to continue.

The Manifestation Determination portion of the 504 MDR opens up additional questions and prompts based on what the 504 MDR team determines.  

Expand or collapse content Was the conduct in question caused by or did it have a DIRECT and SUBSTANTIAL relationship to the student's disability/impairment?

1.  Select either Yes or No in response to, Was the conduct in question caused by or did it have a DIRECT and SUBSTANTIAL relationship to the student's disability/impairment?

2.  Enter in the rationale as appropriate. NOTE:  The prompt will change based on if you selected Yes or No.

Expand or collapse content The conduct in question was the direct result of the school district's failure to implement the current 504 plan.

1.  Select either Yes or No in response to, The conduct in question was the direct result of the school district's failure to implement the current 504 plan.

2.  Enter in the rationale as appropriate. NOTE:  The prompt will change based on if you selected Yes, No (with areas of non-implementation or No, fully implemented.

Expand or collapse content The determination of the 504 Team is that the behaviour subject to discipline is:

1.  Select if the behavior is or is not a manifestation of the student's disability/impairment.

2.  Select Continue to Consent to continue the 504 MDR.

Expand or collapse content Preview/Publish the 504 MDR

1. Print the 504 MDR for review and to provide a copy to the parent(s)/guardian(s)/student.

2. Preview the 504 MDR to ensure the information is correct.

3. To complete the 504 MDR, select Publish. NOTE: This will publish the 504 MDR and it will no longer be editable.
