The Medicaid Consent Page will display any type of Parental Consents that have been entered into Illuminate.

1.  This section is where the One Time Medicaid Consent date gets entered.

2.  If Save is clicked once and then one more time the One Time Parental Consent Date will also show within the New Parental Consent (Annual) section.  Note:  There is nothing wrong if you do or see this.  In addition, this section will display any refusals of parent consent and any historical Annual Parent Consents from before April 2012.

1.  If a date already exists in the One Time Consent Start Date field ensure that it is also below under the New Parental Consent (Annual) as the Most Recent Parent Consent Date.  If it is not select Save.  This will move this date to the section.

2.  Enter the new One Time Parental Consent Date and select save.  Be sure to not leave the page until the throbber stops circling.

2. Enter the date of the refusal to sign the Medicaid Consent.

3. Select No.

4. Click Save.  Wait until the throbber stops circling then select Save again.

Note: This will keep the Medicaid Consent status as No for a year.  After the year has passed the Medicaid Consent Form will be a part of any process until a new date has been entered.

Step One: Complete the steps for How to Show a Parent Does not give Consent.

Step Two:  Enter a date in the To date field for a time in the future when the student will no longer be in school and then select Save.
